Before your training can start, you must fill out these documents. You may have already received links by email for these documents already, but if not, you can download them here. Please follow the directions above each button.

SSI Privacy Policy

All students must have a current medical questionnaire on file to begin their in-water training.

Diver Medical Questionnaire

All students are required to fill out this waiver. Without it, you can't begin in water training.

SSI Liability Waiver

If a student under the age of 18 is participating in training, this youth waiver must be signed by the parent or legal guardian, or the student will be unable to begin training.
